New Year, Same Mistakes: The AI Planning Errors I Keep Seeing

AI planning mistakes are as predictable as January itself. It’s the new year. Companies are announcing ambitious AI plans. By December, most will have a gap between what they planned and what they delivered that they’ll spend Q4 explaining.

I’ve watched this cycle enough times to see the patterns clearly. Here are the planning errors I see most consistently — and how to avoid them before they compound.

Mistake 1: Starting with the Technology Instead of the Problem

The most common error, by far. “We’re going to deploy AI agents” is not a plan. “We’re going to use AI to reduce time-to-resolution for our top 3 product setup issues by 50%” is a plan.

The technology is the tool. The problem is the target. When you start with the technology, you end up force-fitting it to problems where it may not be the right solution. Ask yourself: what specific customer or business outcomes are you trying to change? What’s the baseline? What does success look like numerically? If you can’t answer these before you choose your tools, you’re not ready to plan your AI investment.

Mistake 2: Underestimating the Data Work

AI systems are only as good as the data they’re trained on and connected to. Companies consistently plan the AI deployment and underestimate the data preparation work. They discover mid-project that their knowledge base is 40% outdated, their ticket tagging is inconsistent, their product documentation has gaps. The project slips. The deployment underperforms.

Plan the data work first. Audit your knowledge base. Establish consistent ticket tagging taxonomy. Document the resolution paths for your top issue categories. None of this is glamorous. All of it is prerequisite.

Mistake 3: No Escalation Design

AI systems fail. The question is what happens when they fail. Without deliberate escalation design, the failure mode is: customer gets frustrated, either abandons or calls anyway, agent starts from scratch. You’ve added a step without adding value.

Good escalation design answers: what signals indicate the AI should hand off? What state does it pass to the human agent? How does the human agent know the context without having to ask the customer to repeat everything? This design work happens before deployment, not after.

Mistake 4: Measuring Deployment Instead of Outcomes

“We deployed AI agents in Q1” is not a success metric. Neither is “our AI handles X% of tickets.” These are activity metrics. They measure what you did, not what it produced.

Gartner warned that overreliance on AI metrics could atrophy critical thinking. The AI planning equivalent: measuring AI activity rather than customer outcomes atrophies your ability to evaluate whether the investment is actually working.

Measure: customer satisfaction with AI-handled interactions. First-contact resolution rate. Time-to-resolution. Escalation rate over time. These are outcome metrics. Track them from day one.

Mistake 5: Treating AI as a One-Time Project

AI deployments aren’t projects. They’re capabilities. Projects have end dates. Capabilities require ongoing investment and attention. The AI system you deploy in Q1 will need training updates in Q2, architecture changes in Q3, and a fundamental reassessment in Q4 as you learn what actually works.

Companies that treat AI deployment as a project consistently get worse results than companies that treat it as an ongoing operational capability with a dedicated improvement cycle. Budget for ongoing improvement from the beginning. Build the review cadence in advance.

The One Question That Fixes Most of These

If you’re building an AI plan for 2026, ask one question before you commit to anything: “What will we measure, when will we measure it, and what will we change if the numbers aren’t moving?”

If you can answer that specifically for your AI initiative, you’ve done more planning rigor than 80% of companies. It’s January. You still have time to plan this right. Use it.
